خلاصه يا چکيده : ‭effective studies beside of clinical evaluation for benefits and limitations at least in Iran is clearly needed. Lack of evidence for priority of any of current diets and methods made it difficult for clinical decision and treatment plan for EN support. This study was planned to compare commercial liquid diets with those of HPF used in teaching hospital of Kamyab in Mashhad (University of Medical Sciences). To our knowledge this is the first study in Iran that compares commercial formula and HPF in ICU. The results will have implications for those patients need entral nutritional support. Methods: A total of 70 patients undergoing enteral nutrition were randomly divided into two groups. One group received commercial (standard) and the other received HPF, prepared based on exchange lists (control). Both were fed for 14 days via nasogastric tube feeding. Two formulations were compared by clinical outcomes (length of hospitalization, complications, gastrointestinal tolerance and mortality) and hospital costs using clinical and diet cost analyses. HPF also examined for calorie density, macronutrient, viscosity, osmolality and microbial contamination. Results: Patients in control group received only 45.2-made, hospital blenderized liquids and several commercial formulas have long been used in clinical setting. Counterbalance of clinical benefits and complications with their costs, availability and ease of use may have implications for hospital usage or even in home care settings. Although, commercial formula are available in Iran but using them is yet limited instead, hospital prepared formulas (HPF) are still preferred. There is no standard method for preparing these feedings. To convince of clinicians, insurance companies and hospital officials to use of each of available fluid diets, cost-Background: Enteral nutrition (EN) is crucial for patients that cannot feed orally. Different mixtures have been used for EN and several advantages and limitations have been mentioned for each. Home‬▒‭14.8 percentage of their energy requirement, while the standard group patients received up to 50.5‬▒‭13.6 percentage of their required calories. Macronutrient analysis showed that the amount of carbohydrate intake in control group (p<0.001) were significantly lower than that of the other group. Lengths of hospitalization tended to be lower in standard group (23.0‬▒‭15.3, vs: 29.4‬▒‭22.0 days, P=0.18). Mortality rate at the day of 28 was 8.6 for standard group and 14.3 for control group (P=0.46). According to the average of total cost and hospital days required for each group, cost saving in standard group was at least 361,599 Rial for each day. Conclusion: The result of present study indicates that commercial formula is more effective than hospital prepared formula for patients who admitted to ICU. Standard group had more improved clinical outcomes with lower hospital costs comparing with control ones. These results have implications for those need entral nutritional support, insurance companies and hospital officials and demonstrated the clinical preference of commercial formulas for inpatient clients.
